HOLIDAY SCHEDULE AVAILABILITY FORM

Hey everyone! We need to get your availability/travel as we start to schedule for the holidays. We will coordinate the coverage staffing for the following dates:

- Thanksgiving Wednesday November 23- Sunday November 27

- Christmas/New Years Friday December 23-Sunday January 1

If you are gone outside of these time frames, then you are responsible for finding coverage.

If you plan to leave during these times, please let us know specific dates you will be gone.

If you are able to work, let us know your wishlist availability and we will work it out! We appreciate your willingness to help!
